We have a special program that helps you get care.  The program connects you with team members who can make visits to your home.  They can connect you to groups in your community.  The program is called MemberConnections™.  A MemberConnections team member can come to your home to visit.  You can call to request a visit at 1-866-796-0530 (TDD/TYY (800) 955-8770).  They will talk to you about:

  • Case Management Services
  • Start Smart Baby Program
  • Non-emergency transportation
  • 24 hour nurse line
  • How to choose a doctor (PCP)
  • How to change doctors
  • How to use Sunshine Health services
  • Emergency care and urgent care

MemberConnections representatives will connect you to community social service programs. These programs will help you with food, housing and clothing.

CONNECTIONS Plus

CONNECTIONS PLUS is a program that provides members with free cell phones.  These phones are available to members in Case Management Services.  When you qualify a MemberConnections team member will bring it to your home.  This cell phone has limited use.  Members may use this phone to call:

  • Health Plan Case Manager
  • PCP
  • 24 hour nurse advice line
  • 911
  • Emergency Contact
