How to Enroll

Eligibility is determined by the Florida Healthy Kids Corporation (FHKC). Founded in 1990 by the State of Florida, the FHKC is a not-for-profit organization that provides low-cost health insurance coverage for children ages 5 through 18 through various contracts with managed care companies (like Sunshine Health). It is important that you continue to pay your premium to them regularly and on time so that your coverage is not ended. If you do not pay the monthly premium to Florida Healthy Kids your child’s coverage will end and your child will not be allowed to re-enroll for a minimum of 30 days.

The address for premium payments is:

Florida Healthy Kids Corporation
P.O. Box 31105
Tampa, Florida, 33631-3105.

You may also make your Healthy Kids payments through the Internet (www.healthykids.org/pay) or on the telephone 24 hours a day, 7 days a week (1-800-821-5437).

If you have any questions as to how to pay or how much to pay, you may call Florida Healthy Kids at 1-800-821-5437.

It is important that you read any letters that you receive from Florida Healthy Kids and that you follow any instructions provided in their letters to you.
