Nonprofits can apply now for Community Connections Resource Grants

Sunshine Health wants to help Florida 501(c)(3) organizations that assist their local community with food insecurity, homelessness and other Social Determinants of Health by offering Community Connections Resource Grants (CCRG) for up to $2,000.

If your nonprofit runs a program addressing food, financial assistance, housing or homeless services, or behavioral or mental health services, we encourage you to complete this application. Email the form to SM_FL_CommunityConnections@SunshineHealth.com.

To be eligible, nonprofits must have 501(c)(3) status in Florida. Applications will only be accepted from Mar. 1, 2022 to 5 p.m. Eastern on Mar. 15, 2022. Applicants must answer all of the questions on the form in order to be considered. Please note: some reporting is required.

What are the requirements to apply for a Resource Grant?

  • Organizations must be based in Florida and be registered as a nonprofit 501(c)(3) to apply.

How long does the grant process take?

  • The period for grant applications is March 1, 2022 to March 15, 2022. Awardees will be notified within 15 days after the application process is closed.

How to Complete the CCRG Application:

  • Open the Community Connections Resource Grant Application Form.
  • Respond to all questions in the fields provided.
  • Please type “N/A” in any fields as applicable.
  • Please be sure to fill out the application accurately and completely. Incomplete applications will not be considered.
    • Organization’s Mission – Please include your organization’s mission statement, vision statement, and/or one or two sentences about the purpose behind your organization.
    • Requested Amount – Please enter the grant amount you are requesting. Requests ranging from $1,000-$2,000.
    • Description of Grant – Please provide 100 words or less describing the initiative you hope to fund with your Resource Grant and the anticipated impact of the award to your organization.
    • Objective(s) – Please describe what you hope to achieve through the Resource Grant, and the measurable outcomes you hope to see as a result.
  • Send the completed application as an attachment to your email to: SM_FL_CommunityConnections@sunshinehealth.com.
  • An automatic response email will serve as your confirmation that your email was received.
