Community Co-Stars Incentive Program

Our Community Co-Stars incentive program rewards your Assisted Living Facilities for helping members live in a community setting after they transition from a custodial nursing home.

Note: Providers with capitated agreements are not eligible for incentive programs.

The Community Co-Stars incentive program provides an incentive payment to Assisted Living Facilities (ALFs) for helping Sunshine Health Long Term Care members continue to live in a community setting after they transition from a custodial nursing home stay.

Assisted Living Facility providers will receive $300 per month, for up to 12 months, for each member that remains in the ALF for a minimum of 16 days each month post nursing home transition.

Providers are eligible for an incentive payment when qualified members stay in their facility post nursing home transition for a minimum of 16 days per month and do not readmit to a nursing facility. 

If the member expires during the month, the time that the member remained in the ALF will be considered. The ALF will receive $300 for the month the member passed away if payment criteria was met.

Incentive payments will be made monthly and accompanied by an explanation of payment.

Providers must have an updated W-9 tax form on file with the health plan to ensure no disruptions in payment. To update your W-9, utilize the Provider Demographics Update web form.
