Our Comprehensive Long Term Care (LTC) plan is for people ages 18 and older who have complex healthcare needs and are also eligible for Medicaid.
Our plan offers comprehensive physical and behavioral health services and programs, tools and supports to help you and your caregivers improve your health and quality of life. Our goal is to keep you living in your home or in the community when possible.
Your LTC care manager will help you get the care you need and work with you to develop a person-centered care plan to achieve your unique health goals.
If you have any questions, we’re here to help Monday through Friday, 8 a.m. to 8 p.m. You can call us at any time, even after business hours, to reach our 24-hour Nurse Advice Line.
It is important that we learn about you so we can make sure you get the care that you need. Your care manager will set up a time to come to your home or nursing facility to meet you.
At this first visit, you care manager will tell you about the LTC program and our Plan. She or he will also ask you questions about your health, how you take care of yourself, how you spend your time, who helps takes care of you and other things. These questions make up your initial assessment. The initial assessment helps us learn about what you need to live safely in your home. It also helps us decide what services will help you the most.

Note: Sunshine Health’s Comprehensive Long Term Care members also have Medicaid coverage with Sunshine Health. If you have Long Term Care coverage only, the Medicaid benefits referenced on these pages do not apply to you.
