If you are living with a long-term health issue or disability, now you can receive the care you need at home.
Sunshine Health is a managed care organization (MCO) contracted with the Florida Agency for Health Care Administration (AHCA) to serve Medicaid, Long Term Care, and other program members. Sunshine Health has developed the knowledge to work with Medicaid and Long-Term Care members to improve their health status and quality of life through the quality services to meet the care needs of members.
Since August 1, 2013, Sunshine Health has provided Statewide Medicaid Managed Long Term Care to Medicaid members 18 years or older as defined by CARES to meet the Nursing Facility level of care. You will have a Long Term Care Case Manager working with you to create a care plan and put services in place.
Our goal is to keep you living in the comfort of your home and community. Our approach is to work with you, your care giver, doctors, and others. We want you to have access to the care that you need. Some of the services are; personal care aides, meals brought to your home, respite, and assisted living. You will have a Long Term Case Manager working with you to create a care plan and put services in place.
Your health plan includes a transportation benefit to get to and from your healthcare providers. Use your transportation benefit when travelling between your home, medical appointments, healthcare facilities or the pharmacy. Your transportation benefit is offered at no cost and there is no limit to the number of trips – you can schedule as many trips as you need each year.
LogistiCare administers your transportation benefit. You have two ways to request transportation:

All routine appointments require a 24 hour advance notice and made one (1) day before your appointment date.
Emergency ambulance trips are not arranged through LogistiCare. If a medical emergency ambulance trip is needed, please call 911. Examples of emergency requests include sudden life threatening medical situations, significant trauma, comas, shock, uncontrolled bleeding, respiratory distress, poisoning, drug overdose or any situation where immediate medical relief or treatment is necessary.
In some cases, you may be able to be reimbursed for mileage if a friend or family member drives you to your scheduled appointment. You will need to fill out a mileage reimbursement form (PDF) then mail the completed form to LogistiCare after your visit.
