Authorization Reminders for Skilled Nursing Facilities Servicing LTC Members

On October 1, 2025, Sunshine Health implemented a requirement for Skilled Nursing Facilities (SNFs) to obtain prior authorization for LTC members residing in the facility.

To comply with the requirement, please note the following reminders:

  • Use the latest Inpatient Prior Authorization Fax Form (PDF), which includes the LTC fax number and codes.
  • Use code 101 for LTC Custodial Long-Term Days.
  • Fax SNF LTC custodial authorization requests to our LTC Utilization Management Department at 1-844-416-8319.
  • Submit only one SNF custodial authorization request per member. Multiple submissions for the same member may delay processing.

Email Submissions Ending

Starting December 1, 2025, the Sunshine Health LTC Authorization Department will no longer accept SNF LTC custodial authorizations via email. 

  • All SNF LTC custodial authorization requests should be submitted to the plan via:
  • Sunshine Health will continue to honor SNF LTC custodial authorization requests received through email from the time of October 1, 2025 through November 30, 2025.
