Reminder: Send in Your Provider Termination Requests

Sunshine Health reminds providers that they must quickly notify the health plan when a practitioner separates from a practice, group or facility. This will help the health plan maintain an efficient and productive business relationship with your organization.
Quickly notifying Sunshine Health about terminations will help prevent claim and credentialing issues, support compliance and accreditation standards, and make sure our directories contain the most up-to-date information. That helps our members get the care they need.
Per the Provider Manual (PDF), providers must notify Sunshine Health in writing within 30 calendar days of the departure, retirement, or termination of a rendering provider (physician, advanced practitioner, or other credentialed professional).
By notifying Sunshine Health about terminations on a timely basis, you will help prevent delays during credentialing and recredentialing cycles.
For situations requiring formal termination documentation, providers may also submit termination requests on the letterhead of their practice, group or facility.
Each termination request should include:
- Practitioner’s full name
- Group or practice’s full name
- National Provider Identifier (NPI)
- Taxpayer Identification Number (TIN)
- Effective termination date
- The request must be submitted on the letterhead of the practice, group or facility
How to Make Changes to a Provider Group or Facility
Use the Provider Demographic Updates tool to submit the following changes:
- Practitioner terminations (as applicable)
- Address, contact, or demographic updates
