New Exception Request Form for BA Enrollment Pause

As previously announced, effective October 1, 2025, Sunshine Health has implemented a temporary pause on adding new practitioners to existing Behavioral Analysis (BA) provider groups.* This pause allows us to ensure that our current network of BA providers is fully credentialed and accurately reflected in our systems.
We understand that there may be unique circumstances where an exception is necessary. To support this, we have created a new Exception Request Form for providers who wish to request consideration during the moratorium.
Please complete and submit the form if you believe your request meets criteria for an exception. Each submission will be reviewed on a case-by-case basis, with priority given to areas where a need has been identified. Please do not submit an enrollment request until you have heard back from the health plan on your exception request.
