Nonprofits, apply now for Community Connections Investment Grants

Does your 501(c)(3) need help funding a program that addresses social service needs? Sunshine Health is offering Community Connections Investment Grants (CCIGs) up to $4,000 to support organizations and agencies providing vital social services to their communities.
If your nonprofit runs a program addressing food, financial assistance (rent and/or utilities), housing, or a condition-specific support service (mental health, social isolation, disability support), we encourage you to complete this application and budget form. Email both forms to SM_FL_CommunityConnections@wellcare.com.
To be eligible, nonprofits must have 501(c)(3) status in Florida. Applications will only be accepted from Oct. 15, 2021 to 5 p.m. Eastern on Oct. 30, 2021. Applicants must answer all of the questions on the form in order to be considered. Please note: some reporting is required.

What are the requirements to apply for a CCIG?
- Organizations must provide a social service program in need of funding. Organizations must be based in Florida and be registered as a nonprofit 501(c)(3) to apply.
How long does the CCIG process take?
- The application period is Oct. 15, 2021 to Oct. 30, 2021. Awardees will be notified within 15 days after the application process is closed.
How to Complete the CCIG Application:
- Open the Community Connections Investment Grant Application Form.
- Respond to all questions in the fields provided.
- Type “N/A” in any fields that aren’t applicable to your organization.
- Fill out the application accurately and completely. Incomplete applications will not be considered.
- Organization’s Mission – Please include your organization’s mission statement, vision statement, and/or one or two sentences about the purpose behind your organization.
- Requested Amount – Please enter the grant amount you are requesting for your program (maximum amount $4,000).
- Description of Investment – Please provide 3-5 sentences describing the program initiative you hope to fund with the grant and the anticipated impact of the award to your organization.
- Objective(s) – Please describe what you hope to achieve through the CCIG, and the measurable outcomes you hope to see as a result.
- Send the completed application as an attachment to your email to SM_FL_CommunityConnections@wellcare.com.
