Sunshine Health is offering providers free Clinical Documentation Improvement (CDI) webinars in January to help providers improve their practice’s coding skills. The virtual training sessions are focused on the 2024 CMS Model and ICD-10 Diagnosis Code Updates.

Learn About Continuity of Care (CoC) Appointment Agendas

• The Sunshine Health Continuity of Care (CoC) program is designed to support your outreach to members for annual visits and condition management. This will help us identify members eligible for case management.
• Providers are eligible for a bonus for each completed Appointment Agenda (disease conditions/continuity of care portion only) with verified/documented diagnoses.
• Appointment Agendas are a valuable tool that offers insight and historical diagnosis data into your patients, and clinical services that ensure providers assessing members and all member conditions at least once a year.
• Providers earn bonuses by submitting documentation of current health status (ICD10 codes) and STAR HEDIS/ Medication Adherence Measures (CPT, ICD 10, and NDC codes), including treatment plans for active diagnoses.

Find Appointment Agendas

• Sunshine Health Medicaid (MMA), Ambetter (Marketplace) and Wellcare Complete (Wellcare by Allwell): Providers can find their Appointment Agendas through the Secure Provider Portal via the via the Provider “Analytics” tab.
• Wellcare (Medicare): Providers can access and submit Appointment Agendas via the Wellcare Secure Provider Portal. Completed CoC agendas can also be faxed to Wellcare at 1-813-464-8879.
