Find-a-Provider Directory Accuracy

Thank you for your continued partnership. We remain committed to ensuring our members have access to quality care from valued providers such as yourself.

A key component of ensuring access to care is maintaining and displaying accurate data in our Provider Directory, so that our members can find the right provider in the right location for their healthcare needs. To support that effort, we have contracted with a leading healthcare data accuracy vendor to perform quarterly reviews of our Provider Directory data and ensure that the information displayed to our members is up to date.

The use of our vendor’s patented, proprietary data cleansing algorithms will help us to identify and suppress outdated provider information from our directory, as well as identify potentially new or changed data on a quarterly basis. This new cleansing process will be seamless to you.  Provider attestations are not required, and any necessary suppressions will be implemented automatically.

To see how your data is currently displayed to members in our directory, you may visit our Find-a-Provider tool and search for all applicable practices, locations, or practitioners. Should you find any outdated information, or that your data has been suppressed in error, please follow the steps below. 

Update Your LOAP/Roster

Providers can update their demographic information by downloading and submitting a copy of the LOAP/Practitioner Roster Form (Excel), which is located under our Become a Provider page.

  • LOAP/Practitioner Roster Form (Excel)
    • Please make additions only
    • Do not submit a full roster
    • Ensure you complete the required fields
    • Include most recent hours of operation
    • Confirm if panel (if applicable) is currently accepting new members
    • Download this form every time you use it. Do not save and re-use the file
    • Email your updated LOAP/roster to Sunshine_Provider_Relations@sunshinehealth.com
    • Put the name of your group/practice in the email subject line

After-Hours Availability

As a reminder, after-hours occur between 5 p.m. and 8 a.m. Monday through Friday, and any time on weekends. It’s very important to utilize our List of Affiliated Practitioners (LOAP)/roster form when notifying us of changes to your demographic information, or when adding a new practitioner or location to an existing group or practice.

Download the LOAP/Practitioner Roster Form (Excel) and update your Hours of Operation to include after-hours availability. Email to Sunshine_Provider_Relations@sunshinehealth.com for processing.
