Nonprofits Can Apply for $3,000 Community Connections Grants or up to $15,000 for Storm Relief and Preparation

Sunshine Health is offering Community Connections Grants to 501 (c)(3) organizations that assist their communities with food insecurity, housing and other Social Determinants of Health.  We will award 34 Level 1 Grants of $3,000 to qualifying nonprofit organizations across Florida’s 11 Medicaid regions.

To be eligible, organizations must have 501(c)(3) status in Florida. Applications will be accepted from Aug. 28 to Sept. 20, 2023 at midnight. Applicants must answer all of the questions on the form to be considered. Please note: You will be asked to provide a report on your grant’s outcomes halfway through the grant period and at the end of the period.

Storm Relief and Preparation Grants Up to $15,000: With Hurricane Ian nearly a year behind us, thousands of Floridians are still recovering as a new storm season is underway. Sunshine Health is offering two levels of Storm Relief and Preparation Grants for nonprofits helping people with relief assistance or preparations. Storm relief and preparation can be defined in a number of ways, from providing essential supplies like food and water, to workforce programs for clients who lost their jobs due to a storm.

Level 1 Storm Grant: One-time help of $4,000. Some reporting required.

Level 2 Storm Grant: Programmatic help of $5,000 to $15,000. This level will require detailed reporting.

Application period: August 28–September 20, 2023, midnight

What are the requirements to apply?

Organizations must be based in Florida and be registered as a nonprofit 501(c)(3) to apply.

How long does the grant process take?

Awardees will be notified within 30 days after the application process is closed.

Can I Apply for Both the Level 1 and the Storm Relief/Preparation grants?

Yes, but it’s unlikely we will award you both. We recommend applying for the one that best fits your needs.

Are we eligible for these grants if we won a Maternal Care Grant in March 2023?

While you are not eligible to apply for the $3,000 Level 1 Grant in 2023, you may apply for the Storm Relief and Preparation Grant. Applications will be considered on a case-by-case basis.

Are we eligible to apply if we won a grant in 2022, 2021 or previously?

Yes.

Can the funds be spent on non-Sunshine Health Members?

Yes.

What reporting is required?

For General L1 $3,000 grants and Storm grants up to $4,000, you will be asked to submit a photo of how your money is being used (example, client picking up food at a food pantry), plus a report on the number of people served and outcomes mid-grant and at the end of the grant period.

Storm grants for $5,000 and above will require you to sign a Business Associate Agreement and track and report on Sunshine Health Members served with the grant money (note: you may still use grant dollars for non-Sunshine Health clients).
