Sunshine Health Reminders for Tax Season and Incentive Payments

Sunshine Health has some tips to help providers with W-2 (1099) and W-9 tax forms for tax season and for incentive payments.
W-2 (1099 form) Requests
If you receive a request from Sunshine Health for a copy of your 1099, please email the following information to Reprint1099@centene.com:
- Provider Tax Identification Number (TIN)
- Provider Legal Name
- Tax year(s)
- Provider contact name
- Provider contact phone
- Provider contact email
- Type of request (select one):
- Reprint
- Reconciliation
- 1099 dispute (provider is disputing the 1099 Sunshine Health reconciliation report you received)
W-9 Changes to TIN and/or Name
Send to the Sunshine Health Contracting team by emailing SunshineContracting@sunshinehealth.com
W-9 Changes to Payment Address
- How to Submit: These time-sensitive requests should be submitted via the Provider Demographic Updates Form.
- Best practice: Send us an updated W-9, even if no changes have been made.
- Warning: Failing to comply could result in the withholding or misdirection of payment due to an outdated W-9 being on file.
Incentive Payments
To ensure Sunshine Health has your most current information on file for incentive payments, please submit your W-9 via the Provider Demographic Updates Form.
