Get Rides to Medical Appointments – At No Cost to You

 

To make it easy for you to get healthy and stay healthy, Sunshine Health provides rides to your medical appointments at no cost to you.

There are no limits to the number of trips that you may schedule during the year to take you to medical appointments, healthcare facilities or pharmacies.

Alivi is the transportation service provider we contract with to provide these services to you.

 

To schedule a routine ride, call Alivi reservations at 1-888-863-0248 (TTY 711) between 8 a.m. and 5 p.m. Eastern Monday through Friday. Trip requests must be made 24 hours in advance.

Yes. You, a relative, caregiver or facility staff member can schedule rides.

  • Transportation to and from all your medical appointments. This could be a bus, a van that can transport people with disabilities, a taxi or other kinds of vehicles.
  • Members 18 and older can schedule one round trip per month for non-medical trips like shopping or social events.
  • We cover the following services for members who have no transportation:
    • Out-of-state travel
    • Transfers between hospitals or facilities
    • Escorts when medically necessary

Prior authorization is required for any trips over 100 miles.

Member must not have another means of transportation to qualify. 

Yes. Reimbursement is available per mile. To qualify for reimbursement, you must call Alivi at 1-888-863-0248 (TTY 711) Monday through Friday between 8 a.m. and 5 p.m. Eastern. All reimbursement requests can be made up to 30 days in advance but no later than the day of the appointment. Back dated mileage reimbursements are not allowed.

When you call in your reservation, you should provide the vendor with the name and mailing address of the person to whom the reimbursement is to be made payable. Once the trip is verified, you will be issued a Reference Number. This Reference Number becomes your Mileage Reimbursement Trip/Job number. You will only be reimbursed for trips on your Mileage Reimbursement Trip Log that includes this number.

You must not have another means of transportation to qualify. Gas mileage reimbursement is an option if you have someone that can assist with transportation. Members, spouses or parents/guardians cannot get gas mileage reimbursement.

All Mileage Reimbursement Trip Logs for Long Term Care and Comprehensive members should be mailed to Alivi for payment at:

Each log must be filled out correctly and completely before you will be reimbursed. Fields include the trip date, medical provider name and phone number, physician/clinician signature, and total number of miles driven associated with that particular visit. Each date of service must have a physician or clinician signature in order for the reimbursement to be approved and must be confirmed with the physician’s office before payment will be made.
